Form warning problems

Please help me fix what is causing these warnings from the form on

Warning: mail() [function.mail]: SMTP server response: 550 Sender is not allowed. in C:\Domains\\wwwroot\contacttest.php on line 84

Warning: Cannot modify header information - headers already sent by (output started at C:\Domains\\wwwroot\contacttest.php:84) in C:\Domains\\wwwroot\contacttest.php on line 86

The lines are not numbered in the page html and I can't copy it to paste into a text editor, no can I see anything obvious. I have not modified any code except inserting a google map onto the page, but it still doesn't work when I remove it.

The error indicates that the mail server (in php.ini on the web server) is not properly configured.
This is not directly related to Quick 'n Easy Web Builder. This is a server configuration issue. The form is using PHP's mail() functionality to send emails.
